CDT:
A Lymphedema Treatment Protocol
CDT, or Combined Decongestive Therapy, is
the recommended treatment protocol for lymphedema. It consists of MLD,
compression bandaging, exercise, skin care, and adherence to precautions.
In most cases, for a successful lymphedema treatment plan, compression
bandaging and garments must accompany MLD. When the lymphatic system
is unable to keep up with the movement of fluid, MLD will stimulate
an increased flow for a few hours, but in between therapy, the lymphatic
system needs the compressive support of bandages and garments. The
lymphatic system has no central pump and relies on the support of surrounding
tissue to create a pumping action. If the lymphatic system is compromised,
the natural pressure from surrounding tissues and muscles is not enough.
With long standing edema, the tissue may have lost its natural elasticity.
Bandages and supportive compression sleeves and stockings take up the
slack, and give the lymphatic system the boost it needs.
Special exercises are another component of a thorough lymphedema protocol,
including breathing exercises. Appropriate use of muscles in the affected
areas helps increase the pumping action of the lymphatic system, especially
against the resistance of compression bandaging.
It is important not to cause fatigue, or to become overheated when
exercising, as this will cause further swelling, and detract from good
results.
